There is no confirmation as to what "Before the Fall" means in the context of the story. The only thing we have is that Before the Fall Part 2 will contain an event called "The Battle for Ishgard."

Might it mean "before the fall [of the gate to Ishgard]"?

It's likely referring to a lot of things. There are quite a few plots that are currently open that could have a dramatic climax happening shortly.

Or to put it another way, no character that shows up outside of a main story quest (i.e. Merlwyb in the Armorer questline, Nanamo in the Culinarian questline, etc.) is going to kick the bucket simply because that introduces continuity errors unless they change those questlines posthumously.
That being said, there is still plenty of dramatic tension in terms of what's going to happen to the city's governance, Omega Weapon, the Ascians, and Ishgard.

I can't think of any quests where the Sultana herself shows up that isn't MSQ.

Think back to your favorite movie. The character who died? Probably the one I liked best. Think of that anime where someone in the cast gets killed out of nowhere. That was my favorite. That TV character who bites it in the season finale? Yeah, that was my guy.
