In fact, the devs have written on this, noting that some pieces of gear have lore about where they come from (Darklight, for instance, is all replicas, along with anything you get from Rowena's people) and others don't (Aethereal gear is just "stuff lying around, possibly from other adventurers who didn't make it out").
