As far as things were concerned during 1.0, the only people permitted to come and go as they pleased were Ishgard's various orders of knights and high ranking officials, such as Lord Aymeric. The gates have been barred to all outsiders, and though normal citizens could leave, they would not be allowed back in if I remember correctly.
