Each character you currently have and will make in the future will receive a Fantasia in the mail, and that goes for all future veteran rewards you'll get, and anything else you've ever gotten from things like recruit-a-friend or collector's editions! (All of my characters have/start with three fantasias total xD)
As for how you use one, you just consume it, like a potion or food, remove all your gear (so race- and gender-specific gear aren't a problem), then log out, and then next time you're at character select you can choose to change that character's appearance before logging in.
