As for pregnancy you'd incorporate that into it. So as you'd include something about how Bob can't actually reach the fish because his baby lump is too huge. It's basically just letting people know what your character is doing at any time. Think of it as if you're reading a book. It's not all just a list of speeches, you want to know what the characters are doing as well. Your best bet is to watch other RP's and learn from them.
