My assumption is that the average successful adventurer has a decent quality set of gear and is probably materia-infused, which seems to help a lot with durability and resistance to damage.Â
So, if my character was armed and armoured, I'd probably have his gear deflect or blunt the damage from the first few attacks, then have a few minor flesh wounds, then move to serious wounds. I probably wouldn't have one sword swing or one gunshot take him out, it would take a few.
On the other hand, if he was walking around town in casual raiment or dress clothes, one attack would probably jump straight to serious wound. Maybe not crippling, if I thought I could justify keeping him mobile and trying to get away or counter, but that would depend on the situation and whether or not it was a surprise attack.
I guess my overall view on damage IRP would be more "action/anime-style", where you might take a lot of damage but whether you can keep going or not depends more on your will-power than your physical health. If pressed about that, I'd probably say something about aether, but it's just more interesting/fun than going full realism where one punch to the face will probably knock you out of the fight, or one kick to the leg cripples you for a month.

“My good blade carves the casques of men, my tough lance thrusteth sure.
My strength is as the strength of ten, because my heart is pure.â€
