If no one speaks (as is often the case in the DF) then I'll give it to whoever seemed the most competent (to me).  There have been a few times I haven't given commendations to anyone. Some people are obnoxious or just plain bad. Also something I've found while running with my tank buddy is that healing is a totally thankless job.
