I know I had about 8 in open beta. This require help from "the inside" I think (the inside being Squeenix). Link to comment

One bad character in the name will cause it to fail. In the case of guildmates who are just joining the game, make sure they have actually created and logged in with the character before you try to add them. Link to comment
