  I think permission should be asked if someone wants to have a fight.Injuries same premise but I am usually okay with light to medium injuries anything graver needs to be discussed.

  Depends on the situation would prefer if someone would want to establish family tie in Rp I would prefer to know them. and ask permission for something like that.
